New passport applications forms
New passport application forms were introduced on 1 July 2008.
The Australian Passport Office will to continue accept the old forms until 30 September 2008. After this date, only new forms will be accepted.
If you are in possession of a valid Australian passport or one that has expired less than 12 months ago, you are encouraged to use the streamlined renewal process to replace your passport. Visit www.passports.gov.au or call 131 232 to get a pre-printed renewal form. The only supporting documentation needed to lodge a renewal form is the passport being replaced.
A full application form must be used if your passport expired more than 12 months ago. You will be required to provide the original of either your full Australian birth certificate or Australian citizenship certificate to support your application. With the exception of the renewal process, a previous Australian passport cannot be used with the new forms.
This change is designed to strengthen the identity management process that underpins the Australian passport issuing system. It ensures the names included in replacement passports match those recorded on state and territory births, deaths and marriages registers or citizenship registers.
